Combine the ground beef, sausage, onion, ketchup, breadcrumbs, eggs, garlic powder, Worcestershire sauce, salt, and pepper in a big bowl. Keep mixing until everything looks even and smooth.

Mold the mixture into a loaf shape using your hands, then place it carefully on top of the carrots in the slow cooker.

Whisk together the brown sugar, remaining garlic powder, Dijon mustard, and the rest of the ketchup in the same bowl. Stir until smooth.
